Valgan 450 mg is a potent systemic antiviral agent representing the L-valyl ester prodrug of ganciclovir. Pharmacodynamics: Valganciclovir is a synthetic prodrug of ganciclovir (a nucleoside analogue of 2'-deoxyguanosine). In CMV-infected cells, ganciclovir is initially phosphorylated to ganciclovir monophosphate by the viral protein kinase (UL97), then further phosphorylated by cellular kinases to ganciclovir triphosphate. Triphosphate competitively inhibits viral DNA polymerase and terminates viral DNA elongation.

Pharmacokinetics: Well absorbed from the gastrointestinal tract. Systemic bioavailability of ganciclovir from valganciclovir taken with food is approximately 60%. Plasma protein binding is minimal (1–2%). Eliminated predominantly by the kidneys via glomerular filtration and active tubular secretion. Elimination half-life is approximately 4 hours.

Valgan 450 mg is indicated for:

  • 🔹 CMV Retinitis: Induction and maintenance treatment of cytomegalovirus retinitis in adult patients with AIDS or immunocompromised status.
  • 🔹 CMV Prophylaxis in Transplantation: Prevention of cytomegalovirus disease in high-risk (donor CMV+/recipient CMV-) solid organ transplant recipients (kidney, heart, liver, pancreas).

Administration guidelines for Valgan 450 mg:

  • 🍽️ Take with Food: Administer orally with food to maximize systemic absorption. Tablets should be swallowed whole; do not break, crush, or chew.
  • 👁️ CMV Retinitis (Adults):
    • Induction: 900 mg (two 450 mg tablets) twice daily for 21 days.
    • Maintenance: 900 mg (two 450 mg tablets) once daily.
  • 🏥 Transplantation Prophylaxis: 900 mg once daily starting within 10 days post-transplantation until 100 to 200 days post-transplant depending on the organ type.

Valgan is contraindicated in:

Important drug interactions for Valgan:

  • ⚠️ Mycophenolate Mofetil (MMF): Concomitant use in renal impairment may increase plasma concentrations of both drugs.
  • ⚠️ Zidovudine: Combined therapy increases the risk of severe neutropenia and anemia.
  • ⚠️ Didanosine: Plasma concentrations of didanosine may be significantly increased (risk of systemic toxicity).
  • ⚠️ Nephrotoxic / Myelosuppressive Agents (amphotericin B, cyclosporine, tacrolimus): May enhance renal and bone marrow toxicity.

Valgan exhibits significant teratogenic, mutagenic, and aspermatogenic potential. It is strictly contraindicated during pregnancy and breastfeeding. Females of reproductive potential must use effective contraception during treatment and for at least 30 days post-treatment. Male patients must use barrier contraception during treatment and for at least 90 days following completion of therapy.

Common adverse reactions to Valgan (requires regular complete blood count monitoring):

  • 🔴 Hematologic: Severe neutropenia, anemia, thrombocytopenia, leukopenia.
  • 🟡 Gastrointestinal: Diarrhea, nausea, vomiting, abdominal pain, dyspepsia.
  • 🟡 Central Nervous System: Headache, insomnia, dizziness, paresthesia, convulsions.
  • 🟢 General: Pyrexia, fatigue, dyspnea, increased serum creatinine levels.

Symptoms: Severe bone marrow suppression (profound neutropenia, aplasia), nephrotoxicity, hepatotoxicity, seizures.

Treatment: No specific antidote. Management involves forced hydration, supportive care, and G-CSF administration. Hemodialysis efficiently reduces plasma concentrations of ganciclovir.
