The tenofovir disoproxil + emtricitabine combination is one of the best-known regimens in HIV therapy, and since 2012 also the first approved drug for pre-exposure prophylaxis (PrEP) in HIV-negative people at high risk of infection. The original drug is Truvada (Gilead Sciences). In the Unifarm catalogue, the same combination is available as Tenvir-EM, and as part of triple regimens as Viraday, Trustiva and Tenomtra.

One combination, two different purposes

Tenofovir and emtricitabine are both nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs). HIV is a retrovirus: it carries its genetic information as RNA and must rewrite it into DNA using its own enzyme (reverse transcriptase) before inserting itself into the infected cell's genome. Tenofovir and emtricitabine both resemble the natural "building blocks" of DNA, but they break the chain as soon as the viral enzyme tries to use them — copying stops.

HIV on its own quickly develops resistance to any single drug, so therapy is always built from at least three active substances of different classes at once. Tenofovir and emtricitabine form the "base" of the regimen, to which a third component is added.

A doctor selects the third component and its dose based on test results — resistance to individual drugs varies from patient to patient.

Efavirenz in Viraday and Trustiva belongs to a different class — non-nucleoside reverse transcriptase inhibitors (NNRTIs) — it blocks the same viral enzyme, but differently. Attacking two different sites on the same enzyme at once (plus a separate mechanism in modern integrase-inhibitor-based regimens) is what makes today's HIV therapy so resistant to viral mutation.

For PrEP, the same combination but a different dosing logic

For PrEP, tenofovir with emtricitabine is taken regardless of HIV status — specifically as prevention before exposure to the virus, daily, strictly as prescribed, with mandatory regular HIV testing (usually every 3 months). PrEP does not protect against other sexually transmitted infections and does not replace condoms. Unlike therapy for an already-confirmed infection, PrEP doesn't need a triple regimen — two drugs are enough, because the goal isn't to suppress an already-replicating virus but to prevent it from taking hold after a single exposure.

There is also a newer form of tenofovir — alafenamide (TAF), which places less strain on kidneys and bones at the same effectiveness; in the catalogue that includes combinations based on Tafero. Which tenofovir form suits a given person — accounting for kidney function and other factors — is decided by a doctor.

What matters most

  • An HIV test is mandatory before starting PrEP. Taking tenofovir with emtricitabine with an existing but unconfirmed infection increases the risk of the virus developing resistance to the drug — effectively an under-treated two-drug regimen instead of the required three.
  • Kidney function is checked before starting and periodically during use. Tenofovir disoproxil is cleared by the kidneys and can affect them with long-term use, especially combined with other nephrotoxic drugs.
  • Missed doses reduce protection and raise resistance risk. For both treatment and PrEP, consistent dosing is the key factor in effectiveness; missed doses are the most common cause of treatment failure.
  • Efavirenz (in Viraday and Trustiva) can cause sleep disturbances and vivid dreams in the first weeks — this usually resolves, but it's worth knowing about in advance so treatment isn't abandoned on your own.
