Trustiva is a fixed-dose combination (FDC) antiretroviral medication indicated for the complete single-tablet regimen treatment of Human Immunodeficiency Virus type 1 (HIV-1) infection in adults and adolescents. Each tablet integrates three active pharmaceutical ingredients: Tenofovir Disoproxil Fumarate (300 mg), Efavirenz (600 mg), and Emtricitabine (200 mg). This "one tablet once daily" formulation significantly improves adherence and simplifies long-term management of HIV infection.

Manufactured by Hetero Labs Limited / Hetero Healthcare, a renowned global leader in antiretroviral therapy production based in India, Trustiva is produced in WHO-GMP certified facilities. It complies with high pharmacopeial standards and demonstrates complete bioequivalence to the original reference brand (Atripla).

Key Clinical Advantages of Trustiva:

  • ✅ Single Tablet Regimen (STR): Minimizes pill burden, enhances medication compliance, and reduces the risk of developing viral resistance.
  • ✅ Triple Synergy: Combines two Nucleoside/Nucleotide Reverse Transcriptase Inhibitors (NRTIs/NtRTIs) with one Non-Nucleoside Reverse Transcriptase Inhibitor (NNRTI) for potent viral suppression.
  • ✅ Well-Established Efficacy Profile: Backed by extensive long-term clinical trial data proving sustained undetectable viral load and CD4 cell recovery.
  • ✅ Broad Availability: Provides an accessible, high-quality, cost-effective alternative for global HIV management.

Trustiva is presented as film-coated tablets for oral administration:

  • Presentation & Packaging: Supplied in an opaque HDPE bottle fitted with a child-resistant cap and desiccant, containing 30 film-coated tablets. Each bottle is packaged in an outer printed carton with a patient leaflet.
  • Active Ingredients per Tablet:
    • Tenofovir Disoproxil Fumarate — 300 mg (equivalent to 245 mg of tenofovir disoproxil);
    • Efavirenz — 600 mg;
    • Emtricitabine — 200 mg.
  • Excipients: Microcrystalline cellulose, croscarmellose sodium, lactose monohydrate, magnesium stearate, hydroxypropyl cellulose, sodium lauryl sulfate, film-coating agents (hypromellose, titanium dioxide, polyethylene glycol, colorants).

Pharmacodynamics: Trustiva acts via complementary mechanisms blocking HIV-1 reverse transcriptase enzyme:

  • Tenofovir Disoproxil Fumarate: An acyclic nucleoside phosphonate diester analog of adenosine monophosphate. Intracellularly converted to tenofovir diphosphate, it inhibits HIV-1 reverse transcriptase by competing with natural substrate and causing DNA chain termination.
  • Emtricitabine: A synthetic nucleoside analog of cytidine. Phosphorylated to emtricitabine 5'-triphosphate, it competes with deoxycytidine 5'-triphosphate, halting viral DNA synthesis.
  • Efavirenz: A non-nucleoside reverse transcriptase inhibitor (NNRTI). Binds non-competitively directly to HIV-1 reverse transcriptase, inducing conformational changes that block its catalytic site.

Pharmacokinetics: All three components are rapidly absorbed following oral intake. Peak plasma levels (Cmax) occur at 3–5 hours for Efavirenz, 1 hour for Tenofovir, and 1–2 hours for Emtricitabine. High-fat meals increase Efavirenz exposure; hence, taking on an empty stomach is recommended. Efavirenz is metabolized in the liver via CYP3A4 and CYP2B6 with a long half-life (40–55 hours). Tenofovir and Emtricitabine are primarily excreted unchanged by the kidneys (elimination half-lives of ~17 and ~10 hours, respectively).

Trustiva is indicated as a complete regimen for:

  • 🔹 Treatment of HIV-1 Infection in Adults: As a first-line therapy regimen in antiretroviral-naive adult patients.
  • 🔹 Treatment in Adolescents: Weighing at least 40 kg who can swallow solid oral dosage forms.
  • 🔹 Regimen Replacement: In virologically suppressed patients (HIV-1 RNA < 50 copies/mL for > 3–6 months) with no history of treatment failure or resistance to any component.

Trustiva therapy should be initiated and monitored by an experienced healthcare professional:

  • 💊 Standard Dosage: 1 tablet orally once daily.
  • ⏰ Timing: Recommended to be taken on an empty stomach (1 hour before or 2 hours after food), preferably at bedtime. Bedtime dosing improves tolerability by reducing CNS side effects associated with Efavirenz.
  • 🥤 Administration: Swallow the tablet whole with water. Do not chew, crush, or split the tablet.
  • ⚠️ Renal Impairment: Not recommended in patients with creatinine clearance < 50 mL/min, as dose adjustment cannot be achieved with the fixed-dose combination.

Trustiva is contraindicated in patients with:

  • ❌ Hypersensitivity to Tenofovir, Efavirenz, Emtricitabine, or any excipients.
  • ❌ Severe hepatic impairment (Child-Pugh Class C).
  • ❌ Moderate to severe renal impairment (CrCl < 50 mL/min).
  • ❌ Co-administration with terfenadine, astemizole, cisapride, midazolam, triazolam, pimozide, ergot alkaloids, or St. John's wort (Hypericum perforatum).
  • ❌ Pediatric patients weighing under 40 kg.

Trustiva exhibits important drug interaction characteristics:

  • ⚠️ Protease Inhibitors (PIs): Efavirenz alters plasma concentrations of atazanavir, lopinavir, and darunavir; dose adjustments or ritonavir boosting may be necessary.
  • ⚠️ Nephrotoxic Agents: Concomitant use with drugs affecting renal function (e.g., aminoglycosides, NSAIDs) increases tenofovir-induced renal toxicity risk.
  • ⚠️ Antifungals: Voriconazole concentrations are significantly decreased by Efavirenz; co-administration is contraindicated.
  • ⚠️ Hormonal Contraceptives: Efavirenz reduces efficacy; barrier methods should be added.
  • ⚠️ Rifampicin: Induces Efavirenz metabolism; dose adjustment of Efavirenz may be required.

Considerations for Trustiva during pregnancy and breastfeeding:

  • 🤰 Pregnancy: Avoid in the first trimester unless no alternative exists, due to potential neural tube defect risks associated with Efavirenz. Use in 2nd and 3rd trimesters under medical supervision. Pregnancy testing and reliable contraception are advised.
  • 🤱 Lactation: Breastfeeding is not recommended for HIV-infected mothers to avoid vertical transmission of HIV and potential drug excretion into breast milk.

Potential adverse reactions associated with Trustiva include:

  • 🔴 CNS & Psychiatric: Dizziness, headache, insomnia, somnolence, abnormal dreams, impaired concentration, depression.
  • 🟡 Gastrointestinal: Nausea, diarrhea, vomiting, flatulence, abdominal pain, elevated liver enzymes.
  • 🟠 Dermatologic: Skin rash, pruritus, urticaria. Rarely, severe skin reactions like Stevens-Johnson syndrome.
  • 🟢 Metabolic & Musculoskeletal: Hypertriglyceridemia, hypercholesterolemia, decreased bone mineral density, elevated CPK.
  • 🔵 Renal: Elevated creatinine, proteinuria, renal impairment, rarely Fanconi syndrome.

Management of accidental overdose:

  • Symptoms: Exacerbation of CNS symptoms (severe dizziness, confusion, ataxia), muscle twitches, and GI distress.
  • Treatment: No specific antidote. Supportive therapy, gastric lavage, or activated charcoal. Hemodialysis efficiently removes Tenofovir and Emtricitabine, but not Efavirenz due to high protein binding.

Storage instructions for Trustiva:

  • temperature limits: Store below 30 °C in original tightly sealed container.
  • moisture control: Keep protected from light and moisture. Do not remove desiccant pouch.
  • child safety: Keep out of reach of children.
  • shelf life: 24 months from manufacture date. Do not use after expiration date.

Authenticity features of genuine Trustiva:
