Letrozole is a highly selective third-generation non-steroidal aromatase inhibitor originally marketed under the brand name Femara. It is widely used in gynecologic oncology for the endocrine management of hormone receptor-positive breast cancer in postmenopausal women, as well as in reproductive medicine for ovulation induction.
The mechanism of action of letrozole involves competitive binding to the heme group of the aromatase enzyme (cytochrome P45019), which catalyzes the conversion of androgen precursors into estrogens. This results in profound suppression of circulating estrogen levels (by 97–99%), effectively eliminating the hormonal stimulus for estrogen-dependent tumors.
Clinical utilization of letrozole reduces recurrence and metastatic risks in early and advanced breast cancer settings, while endocrinological applications facilitate follicular development and ovulation in anovulatory infertility (such as polycystic ovary syndrome, PCOS).
Indications
Letrozole is indicated in clinical practice for the following conditions:
- Breast Cancer (Adjuvant Therapy): Treatment of postmenopausal women with estrogen receptor-positive early invasive breast cancer following 5 years of initial adjuvant tamoxifen therapy.
- Advanced Breast Cancer: First-line treatment of postmenopausal women with advanced hormone-dependent breast cancer, or disease progression following prior antiestrogen therapy.
- Ovulation Induction (Off-label): Treatment of anovulatory infertility (e.g., in PCOS) to stimulate follicular growth.
Dosage and administration
Letrozole is intended for oral administration:
- For Breast Cancer: The recommended standard dose is 2.5 mg (one tablet) taken once daily, with or without food. Adjuvant treatment may continue for up to 5 years or until tumor relapse.
- For Ovulation Induction: Typically dosed at 2.5 mg to 5 mg daily, initiated on cycle days 3 through 7 under the supervision of a fertility specialist.
- Monitoring: Patients with uncertain menopausal status must undergo baseline hormonal evaluation (LH, FSH, and estradiol) to confirm postmenopausal status prior to oncological use.
