Bandrone 50 is an advanced antiresorptive agent belonging to the third-generation bisphosphonate class, designed for the prevention and treatment of bone disorders. The active pharmaceutical ingredient is ibandronic acid at a strength of 50 mg formulated as film-coated tablets (Ibandronic Acid Tablets I.P.). The product is developed and manufactured by the Indian pharmaceutical corporation Natco Pharma.

Ibandronic acid binds with high specificity to bone resorption sites, inhibiting osteoclast activity without directly interfering with bone formation. It reduces the incidence of skeletal-related events (pathological fractures), relieves bone pain, and controls malignancy-associated hypercalcemia.

Supplied in packaging containing 10 tablets per carton. Intended for oral administration under the supervision of a qualified oncologist for the management of bone metastases in breast cancer.

Key Clinical Features and Advantages of Bandrone 50:

  • ✅ Bone Protection: Effectively prevents and reduces skeletal complications in metastatic breast cancer.
  • ✅ Pain Relief: Alleviates bone pain associated with tissue destruction, improving patient quality of life.
  • ✅ Convenient Oral Format: 50 mg tablets offer outpatient convenience.
  • ✅ Hypercalcemia Control: Helps regulate serum calcium levels resulting from tumor progression.
  • ✅ Natco Pharma Quality Standards: Manufactured under strict quality assurance standards by a leading oncology specialist.

Bandrone 50 is supplied as film-coated tablets containing 50 mg of ibandronic acid.

Qualitative and Quantitative Composition per Tablet:

  • Active Ingredient: Sodium ibandronate monohydrate (equivalent to ibandronic acid) — 50 mg.
  • Excipients: Lactose monohydrate, povidone, crospovidone, microcrystalline cellulose, colloidal anhydrous silica, magnesium stearate, hypromellose, macrogol, titanium dioxide.

Packaging and Presentation: White to off-white, round, biconvex film-coated tablets with debossing. 10 tablets per carton along with official prescribing information. The outer box and blisters display Bandrone 50 (Ibandronic Acid Tablets I.P.) and Natco Pharma corporate graphics.

Pharmacodynamics:
Ibandronic acid is a potent nitrogen-containing bisphosphonate. It acts selectively on bone tissue by binding to active mineralization sites. The drug inhibits osteoclast activity, leading to reduced bone destruction, lower fracture frequency, decreased hypercalcemia, and reduced need for analgesic therapy.

Pharmacokinetics:

  • Absorption: Rapidly absorbed from the gastrointestinal tract, though absolute bioavailability is low (< 0.6%). Food and beverages (other than plain water) significantly impair absorption.
  • Distribution: Following absorption, rapidly binds to bone tissue or is excreted renally. Plasma protein binding is approximately 85–87%.
  • Metabolism: Not metabolized in the human liver or body.
  • Elimination: Absorbed fraction is eliminated exclusively via the kidneys unchanged. Unabsorbed fraction is eliminated fecally. Terminal half-life ranges from 10 to 72 hours.

Bandrone 50 is indicated in adult patients for:

  • Bone Metastases in Breast Cancer: Prevention of skeletal-related events (pathological fractures, bone pain requiring radiotherapy or surgery) in patients with breast cancer and bone metastases.
  • Malignancy-Associated Hypercalcemia: Management of tumor-induced hypercalcemia (primarily utilized in supportive settings per oncologist guidance).

Bandrone 50 is administered orally once daily at a dose of 50 mg.

Administration Instructions (Strict Requirements to Ensure Absorption):

  • Take in the morning on an empty stomach after overnight fasting (at least 6 hours prior to food or drink).
  • Swallow tablet whole with a full glass of plain drinking water (200–250 ml). Do not use mineral water, juice, tea, or coffee.
  • Patients must not lie down for at least 1 hour (60 minutes) following administration to prevent esophageal irritation.
  • Avoid food, beverages (other than water), and other medications for at least 1 hour post-dose.

Administration of Bandrone 50 is contraindicated in:

  • Hypersensitivity: Known hypersensitivity to ibandronic acid or any excipient.
  • Esophageal Abnormalities: Esophageal abnormalities delaying emptying (e.g., stricture or achalasia).
  • Inability to Stand/Sit Upright: Inability to stand or sit upright for at least 60 minutes.
  • Hypocalcemia: Pre-existing uncorrected hypocalcemia.
  • Severe Renal Impairment: Creatinine clearance < 30 ml/min.
  • Pregnancy and Lactation: Contraindicated in pregnant or breastfeeding women.

Evaluated drug interactions for Bandrone include:

  • Food, Calcium Supplements, and Antacids: Products containing calcium, magnesium, aluminum, or iron significantly impair ibandronic acid absorption. Administer at least 1 hour apart from Bandrone 50.
  • NSAIDs (Aspirin, Ibuprofen): Because both bisphosphonates and NSAIDs may cause gastrointestinal irritation, caution is advised during co-administration.

Pregnancy:
Bandrone 50 is contraindicated during pregnancy. Human data are lacking; animal studies have shown reproductive toxicity.

Lactation:
It is unknown whether ibandronic acid passes into human breast milk. Contraindicated during lactation.

Adverse reactions associated with ibandronic acid are generally mild to moderate:

  • Gastrointestinal Tract: Dyspepsia, nausea, abdominal pain, gastritis, esophagitis, heartburn, dysphagia.
  • Musculoskeletal System: Arthralgia, myalgia, limb pain, jaw osteonecrosis (associated with long-term use and dental procedures).
  • Nervous System: Headache, dizziness, asthenia.
  • Laboratory Changes: Transient hypocalcemia, hypophosphatemia.
  • Other: Hypersensitivity reactions, skin rash.

Symptoms:
Specific acute overdose data are limited. Potential manifestations include gastrointestinal upset (heartburn, esophagitis, nausea) and hypocalcemia (reduced serum calcium).

Treatment:
No specific antidote exists. Administration of milk or antacids to bind the drug in the GI tract, symptomatic care, and intravenous calcium administration if necessary.

Storage Requirements for Bandrone 50:
