Natdecita is a potent antineoplastic cytostatic agent belonging to the hypomethylating class (pyrimidine nucleoside analogue). The active pharmaceutical ingredient is decitabine at a strength of 50 mg per vial as a lyophilized powder for solution for infusion (Decitabine Lyophilized for Injection). The product is developed and manufactured by the leading Indian pharmaceutical corporation Natco Pharma.

Decitabine (5-aza-2'-deoxycytidine) incorporates into DNA and selectively inhibits DNA methyltransferases (DNMT), leading to DNA hypomethylation and subsequent demethylation of tumor suppressor gene promoter regions. This restores silenced tumor suppressor gene expression, regulating cell differentiation and inducing apoptosis in malignant cells during the S-phase of the cell cycle.

Supplied as a sterile single-use lyophilized vial containing 50 mg decitabine (1 vial per box). Intended exclusively for intravenous infusion following reconstitution and dilution. Administered under strict supervision of a hematologist or oncologist in specialized hospital settings.

Key Clinical Features and Advantages of Natdecita:

  • ✅ Hypomethylating Mechanism of Action: Restores normal tumor suppressor gene expression and cellular differentiation control.
  • ✅ Standard of Care in MDS & AML: Highly effective in elderly patients or those unsuited for intensive induction chemotherapy.
  • ✅ Targeted S-Phase Activity: Promotes apoptosis and differentiation in pathologic bone marrow blast cells.
  • ✅ Convenient Dosing Unit: The 50 mg vial strength allows convenient dosing calculations per body surface area.
  • ✅ Natco Pharma Quality Standards: Manufactured under rigorous quality control systems by a global leader in specialized oncology pharmaceuticals.

Natdecita is supplied in the form of a lyophilized powder for solution for infusion.

Qualitative and Quantitative Composition per Vial:

  • Active Ingredient: Decitabine — 50 mg.
  • Excipients: Potassium dihydrogen phosphate, sodium hydroxide and/or hydrochloric acid (for pH adjustment).

Packaging and Presentation: White to off-white lyophilized powder or cake. Supplied in a single-use clear glass vial containing 50 mg decitabine, sealed with a rubber stopper and an aluminum flip-off cap. Each vial is packaged in an outer carton displaying Natdecita (Decitabine Lyophilized for Injection) 50 mg along with Natco Pharma corporate graphics.

Pharmacodynamics:
Decitabine is a deoxycytidine analogue. Its mechanism of action is dual: at lower doses, it induces DNA hypomethylation via DNMT inactivation, re-activating tumor suppressor genes and cellular differentiation pathways. At higher doses, direct cytotoxicity predominates via DNA integration and synthesis inhibition, precipitating cell death.

Pharmacokinetics:

  • Distribution: Following IV infusion, decitabine rapidly distributes throughout body tissues and fluids. Features a wide distribution volume. Plasma protein binding is minimal (< 1%).
  • Metabolism: Rapidly metabolized in the liver, granulocytes, and other peripheral tissues by cytidine deaminase via deamination into an inactive metabolite.
  • Elimination: Features a very short elimination half-life (~35–47 minutes). Excreted primarily via the kidneys as inactive metabolites.

Natdecita is indicated in adult patients for:

  • Myelodysplastic Syndromes (MDS): Treatment of patients with all MDS subtypes (including refractory anemia, refractory anemia with ringed sideroblasts, refractory anemia with excess blasts, and chronic myelomonocytic leukemia).
  • Acute Myeloid Leukemia (AML): Treatment of newly diagnosed primary or secondary AML in adult patients (aged 65 years and older) who are not candidates for standard intensive induction chemotherapy.

Natdecita is administered strictly via intravenous infusion under expert hematologist supervision.

Reconstitution and Dilution Instructions:

  • Reconstitution: Reconstitute the 50 mg vial aseptically with 10 ml of Sterile Water for Injection (yielding a 5 mg/ml concentration).
  • Dilution: Immediately dilute the reconstituted solution in 0.9% Sodium Chloride or 5% Dextrose injection to a final concentration of 0.1 to 1.0 mg/ml.

Dosing Regimen:

  • 3-Day Regimen: 15 mg/m2 body surface area IV over 3 hours every 8 hours (3 times daily) for 3 consecutive days every 6 weeks.
  • 5-Day Regimen: 20 mg/m2 body surface area IV over 1 hour once daily for 5 consecutive days every 4 weeks.

Administration of Natdecita is contraindicated in:

  • Hypersensitivity: Known hypersensitivity to decitabine or any inactive excipient.
  • Severe Hepatic or Renal Impairment: Severe functional impairment of liver or kidney function.
  • Pregnancy and Lactation: Contraindicated in pregnant or nursing women.
  • Pediatric Population: Safety and efficacy in individuals under 18 years of age have not been established.

Evaluated drug interactions for Natdecita include:

  • Myelosuppressive Agents: Co-administration with other chemotherapy or myelosuppressive agents compounds bone marrow suppression risks.
  • Cytidine Deaminase Substrates: Competitive metabolism via cytidine deaminase may alter pharmacokinetics when co-administered with nucleoside analogues (e.g., gemcitabine, cytarabine).

Pregnancy:
Natdecita is strictly contraindicated during pregnancy due to potent embryotoxic, mutagenic, and teratogenic risks. Male and female patients must use effective contraception during therapy and for at least 3–6 months post-treatment.

Lactation:
Breastfeeding is strictly prohibited during decitabine therapy.

The most frequent and clinically relevant adverse reactions reported with decitabine include:

  • Hematologic Toxicity (Very Common): Severe neutropenia, thrombocytopenia, anemia, febrile neutropenia, hemorrhagic events.
  • Infections: Pneumonia, sepsis, fungal and bacterial infections.
  • Gastrointestinal Tract: Nausea, vomiting, diarrhea, constipation, stomatitis, abdominal pain.
  • General & Nervous System: Pyrexia, fatigue, headache, insomnia, peripheral edema.
  • Laboratory Changes: Hyperbilirubinemia, transient hepatic transaminase elevations.

Symptoms:
Decitabine overdose manifests as profound bone marrow suppression (prolonged neutropenia and thrombocytopenia), severe infectious complications, and mucositis.

Treatment:
No specific antidote exists. Supportive and symptomatic care should be initiated: blood product transfusions, granulocyte colony-stimulating factors (G-CSF), broad-spectrum antimicrobial cover, and vital sign monitoring.

Storage Requirements for Natdecita:

  • 🌡️ Temperature Control: Store in a dry, light-protected place at temperatures not exceeding 25 °C (77 °F).
  • ☀️ Light Protection: Keep vial in the outer carton to protect from light.
  • 💧 Reconstituted Solution: Infusion solutions should be administered immediately after preparation. Diluted solutions prepared with cold fluids may be stored at 2–8 °C (36–46 °F) for up to 7 hours before infusion.
