A weekly shot or a tablet every morning: Rybelsus and Ozempic
The Ozempic pen and the Rybelsus tablets share one substance: semaglutide. The same molecule is in the Semaxen tablets. But a shot and a tablet are two different ways to take the same thing. That changes almost everything else: how often you take it, how many milligrams sit on the pack, whether you can eat at once, and whether you need a fridge.
This article does not replace a doctor and does not pick a dose. It is for people whose endocrinologist has already named semaglutide. Why an ordinary semaglutide tablet would not survive the stomach
Semaglutide is a peptide, a protein molecule. Swallow it without protection, and stomach acid and enzymes take it apart like ordinary food. That is why most medicines of this kind come as injections.
Rybelsus adds a protection called SNAC (sodium salcaprozate) to the tablet. For a short time it lowers acidity right around the tablet. Then some of the substance can pass through the stomach wall into the blood.
Even then only about 1% of the tablet reaches the blood. The rest is destroyed. That is why Rybelsus packs show milligrams: 3 mg, 7 mg, 14 mg. The Ozempic pen uses other numbers: 0.25 mg, 0.5 mg, 1 mg, 2 mg. That does not mean the tablet is ten times stronger. Simply less of the substance reaches the blood from a tablet than from a shot.
Four rules for taking Rybelsus
If less than 1% reaches the blood, any slip from the instructions can wipe out that day’s effect. The exact steps are on your own pack. They are usually these.
- On an empty stomach right after waking. There should be no food or drink in the stomach.
- Little water. Usually no more than 120 ml, about half a glass of still water. A large glass dilutes SNAC, and the acid destroys the semaglutide again.
- Swallow whole. Do not chew or split the tablet: the coat keeps the SNAC protection.
- Wait at least 30 minutes, better 45–60. In that window do not eat, drink tea or coffee, smoke, or take other medicines. The longer you wait after the tablet with no food or drink, the more of the substance can reach the blood.

Blood sugar, weight, and why the shot is often more stable
The PIONEER trials compared the tablet and the injection for blood sugar. The top Rybelsus dose of 14 mg is often close to a mid Ozempic dose of 0.5–1 mg for lowering HbA1c. That is not a swap chart and not a reason to convert milligrams yourself.
For weight loss, shots more often go deeper, especially at high doses such as Wegovy 2.4 mg. The reason is simple: a shot puts roughly the same amount into the blood every time. With a tablet, more or less may reach the blood from day to day: how much water you drank, how heavy dinner was the night before, whether you kept the pause.
Whether a given person needs a shot or a tablet is the endocrinologist’s call. In the Unifarm catalogue the pen is Ozempic 1 mg, and the tablets are Rybelsus 7 mg and Semaxen 7.
Can you switch from a shot to a tablet
Yes, a switch is sometimes needed: a long trip without a fridge, or fear of the needle. It is not a DIY pack change.
How it is usually done under a doctor’s control:
- Pen to tablets. Rybelsus starts the day after the weekly injection was due.
- Tablets to pen. The first injection is the day after the last tablet.
Which milligrams to use after the switch is only the endocrinologist’s job. You must not equate 0.5 mg Ozempic with 7 mg or 14 mg Rybelsus on your own.

The fridge and the parcel
