A plateau on tirzepatide or semaglutide: why the scale stopped and do you need a higher dose now?
A plateau means the weight barely drops for several weeks. It happens on Mounjaro KwikPen (tirzepatide) and on semaglutide: the Ozempic pen and the tablets Rybelsus and Semaxen. First the kilos leave quickly, then the scale stays still for two, three or four weeks. Many people panic and want a higher dose at once.

Most of the time the medicine has not stopped working. The body simply burns less energy because it already weighs less than at the start.
1. Why the weight slows down
A 2–4 week pause after a good drop is common. It is not a signal to take the next pack at once. For Mounjaro, 2.5 mg and 5 mg are different catalogue items. People do not switch them on their own.
- Less weight, less burn. An 80 kg person needs fewer calories than a 95 kg person, even when sitting still. After kilos have already gone, the same plate may only hold the current weight.
- Water leaves first. In the first weeks the body uses the carbohydrate store in muscle and liver. Water leaves with it. Then that store settles, and the scale slows even if fat is still leaving.
- The body gets used to the new number. Hunger and energy use adjust to the new weight. That is protection, not a failure of treatment.
2. Why you should not raise the dose yourself

- Doses are not endless. Tirzepatide and semaglutide have an upper limit. If the dose goes up at every pause on the scale, the doctor has fewer options later when a real increase is needed.

3. The scale is stuck: what to check before talking about dose
The scale shows one number. Waist size, bowel habits and food often say more.

4. What you can do without changing the dose
If the weight has not moved for more than three weeks, look first at food, water, sleep and walking. This does not replace a visit to the doctor.
- Protein. Doctors often discuss about 1.2–1.5 g per kg of the weight you are aiming for. If kidneys or other illness limit protein, only the doctor sets the amount.
- More walking. An extra 2,000–3,000 steps a day or 20 minutes of easy swimming often helps without a hard gym.
- One or two slightly fuller days. Once or twice a week you can eat a little more complex carbohydrate and protein. The body is then less likely to save energy. That is not a reason to go to fast food.
- Sleep. Under 7 hours and strong stress hold water and slow fat loss.
When to talk to the doctor about a higher dose
The next dose is chosen by the treating endocrinologist. It is worth raising the topic if all three are true at once:
- more than 4 weeks with no change on the scale or in measurements;
- food, water and walking are actually kept;
- strong hunger is back, and a small plate no longer brings fullness.
