Behind the media noise, blogger reviews and fitness trends a dangerous social idea grew: that complex endocrine medicines can be self-prescribed from a non-specialist site “in one click”.

1. Cultural context: from a clinical breakthrough to “no-prescription” biohacking
Modern culture wants fast, simple answers. When dramatic weight-loss stories filled the feed, many people formed a false picture: that semaglutide such as Ozempic and tirzepatide such as Mounjaro are a handy stand-in for diet and sport — a high-powered supplement.
A huge grey market followed. Telegram channels, marketplaces and cottage web shops began offering peptide molecules to anyone, as if they were household goods. What dropped out of view is that incretin mimetics are powerful prescription medicines. They change how the hypothalamus, pancreas, liver and the whole gut work.

2. Anatomy of the grey market: what sits behind a “cheap” vial
When someone buys outside the official pharmacy channel and clinical control, they meet risks selling pages do not mention.
Lyophilisates and cottage synthesis
A large share of the unofficial market is not factory pens such as Ozempic or Mounjaro KwikPen 2.5 mg, but vials of dry powder (lyophilisate) from unnamed Asian plants sold as “research chemicals”.
- Impurities and heavy metals. Informal synthesis rarely cleans a peptide chain well. Leftover reagents, technical isomers and traces of heavy metals can cause severe toxic and allergic reactions.
- Bacteriostatic water. The buyer is told to reconstitute the powder at home. The slightest break in sterility turns the injection into a culture medium — with abscesses, phlegmon and systemic inflammation as real risks.
- Unstable dose. In cottage vials the real amount of active substance can miss the label by 30–50% either way.
Semaglutide tablets — Rybelsus 7 mg and Semaxen 7 — are not a “safer DIY swap” for a pen. You must not guess a tablet in place of a pen: different form, different absorption, different milligram steps.
3. The physics of degradation: why the cold chain is not paperwork
Semaglutide and tirzepatide are complex protein structures (peptides). Their three-dimensional shape is extremely sensitive to the environment.
To keep biological activity these molecules generally need a continuous +2 to +8 °C range from the plant to the injection. The exact rule is always read from your own pack: the Ozempic and Mounjaro KwikPen pens show it on the product card; Rybelsus and Semaxen tablets are often stored differently.
- Heat. If a parcel rides in an ordinary van or sits in a hot car, peptide bonds break (denaturation). At best the medicine becomes useless “water”. At worst the protein folds into atypical clumps that can trigger a massive immune response.
- Freezing. Going below 0 °C is also forbidden. Ice crystals physically tear the peptide. Not the fridge door, and never the freezer.
